Enterococcus Biology and Vancomycin Resistance Mechanisms in Microbiology

Enterococcus species (specifically *E. faecalis* and *E. faecium*) constitute a distinct taxonomic group within Gram-positive, catalase-negative gamma-hemolytic bacteria characterized by intrinsic salt tolerance at 6.5% NaCl growth conditions and membership in the Lancefield Group D streptococci. The central theoretical mechanism defining their clinical significance is the acquisition of the *van* gene variant, which alters the terminal dipeptide sequence of peptidoglycan precursors from d-Ala-d-Ala to d-Ala-d-Lactate; this molecular substitution prevents vancomycin binding while preserving cell wall integrity and conferring resistance. Consequently, Vancomycin-Resistant Enterococcus (VRE) represents a paradigm shift in nosocomial infection control due to the emergence of multi-drug-resistant pathogens that compromise standard therapeutic options for serious intra-hospital infections such as endocarditis and urinary tract infections.
