Margin Status and Resection Technique in Duodenal Neuroendocrine Tumor Recurrence

Resection margin status and the technique used to remove a tumor are two candidate prognostic factors for local recurrence following endoscopic removal of gastrointestinal neuroendocrine tumors, and their relative and independent contributions to recurrence risk can be evaluated through cohort follow-up. The underlying principle is that deeper planes of resection (e.g., submucosal dissection or mucosal resection en bloc) achieve more complete tumor clearance than superficial techniques (e.g., cold forceps biopsy), such that recurrence risk is influenced by the anatomic depth and completeness of excision independent of margin positivity alone, implicating additional biological factors (such as tumor grade) in recurrence.
