Medication Management for Caregivers

Older adults often take five or more daily medications, and dosing errors are a leading cause of preventable hospitalization in this group. Caregiver medication management means keeping a single current list of everything taken — including over-the-counter drugs and supplements — using a pill organizer or blister packs, giving doses at consistent times, and never adjusting or stopping a medication without the prescriber. Store medications as labeled, watch for new symptoms after any change since side effects in elders often masquerade as confusion or falls, and bring the full list to every appointment.
