Nephron Loss and Uremia in Chronic Kidney Disease Pathophysiology

Chronic kidney failure (chronic kidney disease) is defined as an irreversible loss of nephrons, the functional filtering units of the kidney, resulting in progressive decline of glomerular filtration rate (GFR) and eventual retention of metabolic waste products (uremia). Its pathophysiology centers on a compensatory feedback mechanism — hyperfiltration in surviving nephrons after initial nephron loss — that itself accelerates sclerosis and further nephron death, forming a self-perpetuating cycle. This places chronic kidney disease within renal medicine/nephrology as a progressive, multi-etiology syndrome whose end-organ consequences extend into fluid-electrolyte balance, acid-base regulation, endocrine function (renin, aldosterone, calcitriol, erythropoietin), and mineral/bone metabolism.
