Repositioning for Pressure Injury Prevention

A person who cannot shift their own weight must be repositioned on a regular schedule — commonly every two hours in bed and every hour seated — to relieve pressure over bony points before tissue is damaged. Effective repositioning alternates among back and side-lying positions, uses pillows to keep bony areas like ankles and knees from pressing together, and keeps the head of the bed low to reduce sliding shear. Use the log roll and draw-sheet techniques to move the person without dragging skin across the sheets. Pair every repositioning with a quick skin check over heels, hips, and tailbone, because prevention and early detection are the same routine.
