Secondary Hypertension: Causes and Patient Types in Cardiovascular Pathology

Hypertension is classified as primary, driven by sodium retention that increases intravascular volume and preload (per the Frank-Starling relationship, raising systolic blood pressure), versus secondary, arising from an identifiable underlying cause and following demographic and pathophysiologic patterns distinct by sex and age. Secondary causes include metabolic syndrome-associated atherosclerosis in obese male patients, estrogen-related prothrombotic states and fibromuscular dysplasia in younger females, and atherosclerotic renal artery stenosis in the elderly, the latter triggering renin release from the juxtaglomerular apparatus in response to decreased afferent arteriolar perfusion, activating the renin-angiotensin-aldosterone system; this belongs to cardiovascular pathology/pathophysiology, specifically the mechanisms underlying blood pressure regulation and its disorders.
