The spinal cord comprises three primary functional pathways that mediate distinct sensory and motor modalities: posterior columns for discriminative touch and proprioception (ipsilateral), corticospinal tracts for voluntary fine-tuned movement (upper motor neurons, ipsilateral until decussation in the brainstem or lumbar enlargement), and lateral spinothalamic tract for pain and temperature sensation which crosses to the contralateral side 2–3 segments caudally after entering the cord. The theoretical framework defines specific lesion syndromes based on unilateral hemisection patterns where ipsilateral upper motor neuron deficits, ipsilateral loss of fine touch/proprioception, and contralateral loss of pain/temperature occurring two to three spinal levels below the lesion constitute Brown-Séquard syndrome.
