Thyroid Gland Anatomy and Its Relationship to the Recurrent Laryngeal Nerves
The thyroid gland (right lobe, left lobe, and connecting isthmus) has a critical anatomic relationship to the recurrent laryngeal nerves, which run within the tracheoesophageal groove and, as branches of the vagus nerve, innervate all intrinsic laryngeal muscles except the cricothyroid; the left and right recurrent laryngeal nerves differ anatomically in their looping paths (left around the aortic arch near the ligamentum arteriosum, right under the right subclavian artery). This anatomic proximity underlies the clinical risk of recurrent laryngeal nerve injury during thyroidectomy, producing hoarseness, while thyroid pathology such as goiter can independently compress the nerve, airway, and esophagus to cause hoarseness, coughing, and dysphagia.
